1. Our Purpose and Your Journey
The primary purpose of Family Promise of Puget Sound’s shelter program is to provide comprehensive services and support to families who are committed to attaining and maintaining stable residence in Pierce County. Our services are designed to offer more than just temporary shelter; they include dedicated assistance in obtaining stable housing, securing sustainable income, and accessing essential resources that will help you and your family achieve and maintain long-term self-sufficiency.
Prior to your stay at Family Promise of Puget Sound, your family will be required to meet with a member of our Case Management Team.
2. Your Active Participation: A Condition of Stay
The purpose of your initial meeting with the Case Management Team, and all subsequent sessions, is crucial for your success. During these meetings, we will work collaboratively to:
* Establish Housing Stabilization Plans: Develop a personalized plan tailored to your family’s unique needs and goals for securing stable housing.
* Complete Assessments: Gather necessary background information and complete assessments to understand your strengths and areas where you might need support.
* Overcome Barriers: Discuss strategies and resources to help you overcome barriers to housing stability and self-sufficiency.
* Establish Goals: Set clear, achievable goals that will assist your family in transitioning from Family Promise of Puget Sound’s shelter as soon as possible.
Your signature below indicates that you understand that these goals, meetings, and your active participation in the program are essential for your progress, represent your request for shelter, and are a condition of your stay at Family Promise of Puget Sound’s shelter.
3. Acknowledgment and Agreement
My signature below certifies that I agree to be an active and engaged participant in my housing plan. Furthermore, my signature certifies that I have read and understand these Statements, Policies, and Agreements, and I agree to abide by all the rules and expectations contained within them in order for my family to stay at the Family Promise of Puget Sound shelter. I confirm that I have been given a copy of these documents for my later reference.
