What is FCS?

Foundational Community Supports (FCS) is a program designed to help individuals and families with complex health needs find stable housing and meaningful employment. We believe that everyone deserves a safe place to live and the opportunity to thrive. Our team walks alongside you, providing the tools and advocacy needed to achieve long-term stability.

Supportive Housing

We help you find and keep a home that meets your needs. Our housing specialists assist with:

✔ Negotiating with landlords and understanding leases
✔ Connecting to community resources
✔ Skills for maintaining a stable home

Supported Employment

We support you in finding a job that matches your skills and interests. Our employment team assists with:

✔ Resume building and interview preparation
✔ Job searching and employer outreach
✔ On-the-job coaching and support
✔ Career planning and advancement

Who is Eligible?

FCS services are available to individuals who are Medicaid-eligible and have complex needs that create barriers to housing or employment. This may include:

People experiencing chronic homelessness or housing instability.

Individuals with mental health or substance use challenges.

People with complex physical health needs requiring daily support.

Those needing assistance to obtain and maintain employment.
