Initial Contact & Consultation

Our digital gateway provides “In-Kind Leverage,” allowing us to offer immediate resources and referrals to any family who contacts us, regardless of their current housing status.

Our systems are strictly HIPAA and FERPA compliant. We utilize advanced encryption and “least-privilege” access controls. Information is shared only with explicit consent to ensure family safety and dignity.

Access For All

Technology acts as a force multiplier. Because our intake is digital and streamlined, staff can provide service navigation to families and individuals in crisis, even when physical shelter beds are at capacity.

Initial Capture

Families and individuals choose their level of engagement. Simple “Short Forms” trigger rapid response, while “Full Consultations” begin the deep dive into specialized support services.

Phase 2

Coordinated Entry (CE) Screening

Conducted at the Parkland Consultation Center to assess urgent needs for families and individuals experiencing homelessness.

The Assessment

Data points include housing history, family vulnerabilities, and immediate resource gaps. This is done with a trauma-informed lens to ensure comfort for families and individuals.

Resource Matching

Screening data automatically filters families and individuals into appropriate queues for shelter, rapid re-housing, or permanent supportive housing.

Phase 3

Shelter Hub Management

A real-time dashboard that synchronizes family needs with available physical space across our network.

Dynamic Inventory

Staff monitor unit availability, bed counts, and specialized family requirements (e.g., ADA compliance) in a live, secure environment.

Reservation System

Once a family is matched, the system secures their space, ensuring they have a safe place to land the moment they arrive.

Phase 4

Intensive Case Management

The “Path to Housing Stability” plan becomes the central living document for a family’s success.

Ash Nazg Integration

Our AI-assisted documentation tool captures case notes with incredible accuracy, allowing case managers to spend 80% more time on face-to-face family support.

Employment Search Tool

Integrated job search modules help families and individuals find living-wage opportunities that match their skills and commute capabilities, directly within their case plan.

Long-Term Impact Tracking

Measuring success over years, not just weeks, to prove the FPOPS model works.

Two-Year Follow-up

We maintain contact for 24 months post-housing to track stability and offer light-touch support if hurdles arise.
